Welcome to the South West LHIN
The South West LHIN is leading the way in transforming the health system to better meet the needs of our communities and to ensure a healthier tomorrow. Transformation means collaborating with health service providers – long-term care homes, community support services and hospitals – in redesigning effective approaches in the delivery of health services and to integrate services for more seamless care. Transformation also means setting performance goals and ensuring accountability in meeting these goals.
To guide this transformation we have a three-year strategic plan, the Integrated Health Service Plan (IHSP) 2013-2016. The IHSP outlines numerous programs and initiatives to help people live healthy, independently and safely at home. Click here for more information on the IHSP. On our website you will find information on the progress being made and plans underway to improve the health system for our communities.
Community Funding Announcement in London
Deb Matthews, the Minister of Health and Long-Term Care announced new funding for the community sector at an event in London on November 29. The event, held at Participation House's Project Hope site was well attended by providers and their families to hear that almost $21 million is being made available in the South West LHIN.

Electronic Medical Record (EMR) Needs Assessment Survey
Primary care Physicians, Nurse Practitioners and Physician Assistants in the South West LHIN are invited to participate in the Partnering for Quality (PFQ) Primary Care Electronic Medical Record (EMR) Needs Assessment. Your input is critical to assist the PFQ Program in developing a regional primary care strategy to support your EMR needs to improve quality and chronic disease management in your practice and across the South West LHIN.
Click here to go to the online Primary Care EMR Needs Assessment. The online assessment will take approximately 15 minutes to complete and will be available until Tuesday December 10, 2013.

French Language Services Toolkit
The Erie St. Clair and South West Local Health Integration Networks (LHINs) are now able to offer a French Language Services (FLS) Toolkit to support designated and identified health service providers in delivering quality services in French.
You will find practical tools and basic information on the Francophone community, active offer of French language services, implementation of French language services, human resources, training, translation and interpretation and supporting legislation.
