Health System for All graphicMonitoring and reporting performance is important not only to communicate and demonstrate how the LHIN is achieving an integrated health system for all, but also as a sign of our commitment to accountability and transparency to our community.  
Quality Care, Improved Health, Better Value—these are the triple aim goals of the South West LHIN to improve health outcomes, the experience of care, and value for money, as outlined in our Integrated Health Services Plan 2016-19 (IHSP).

 

Progress will be monitored using three Report on Performance monitoring tools: 

 
 
 

How are we performing?

Report on Performance Scorecard and Big Dots

A set of four high-level or "Big Dot” measures, 21 home and community care service delivery measures, and 25 system measures have been selected to track progress and demonstrate how we are doing against our strategy, and stated priorities and goals. While these measures and Big Dots align to the LHIN’s triple aim system goals, they do not summarize or reflect all of the LHIN’s initiatives.

 


Report on Performance Summary Reports (Priority Summaries)

Performance is measured quarterly against seven priority areas and key objectives identified within the IHSP. Click on each of the IHSP priorities below for one-page summaries of performance and actions underway to impact performance. 
   
 
Big Dots Stronger primary health care that is linked with the broader health care system Optimized health at home, in long- term care and in other community settings
Supporting people in preventing and managing chronic conditions

Timely access to hospital-based care at the LHIN-wide, multi-community, and local level


    A rehabilitative approach across the care continuum
    People with life-limiting illnesses and their families at the centre of hospice palliative care


    Report on Performance e-Tool

    Our Report on Performance e-Tool is an interactive web-based tool. The data is fully customizable and can be downloaded or shared. Data is updated quarterly and will be available the first Wednesday of September, December, March, and June. This tool supports the drill-down of performance measures to the LHIN, sub-region (where available), and health service provider level to support detailed analysis.
