Shaping care delivery with patients and families

London, ON – In fulfilling our commitment to partnering with patients, families and caregivers to shape care delivery, the South West LHIN is pleased to announce the inaugural Co-chairs of the South West Patient and Family Advisory Committee.

Co-chairs Nadia Tahir from London Middlesex and Barb West-Bartley from Grey Bruce will work alongside other committee members to provide advice to the LHIN based on the patient, caregiver, and family experience.

The Patient and Family Advisory Committee is a 16-member group that will advise, collaborate and co-design an approach to patient involvement in transforming the health care system. Through the Committee’s work, community members will have the ability to not just know about the decisions that affect them, but be involved in their development. The committee had its inaugural meeting on October 18, 2017, and is now working on establishing a work plan for the group.


To provide coordinated care for patients, the South West LHIN has an enhanced role in creating more effective service partnerships, greater equity, and is establishing new committees with a stronger patient/family/caregiver voice that can advise on health care system priorities and drive change locally.

Ten of the patient, family and caregiver partners participating in the committee will also be members of a Sub-region Integration Table; these tables are focused on identifying, planning and making recommendations for local priorities while driving change through innovation and collaboration. Five other committee members will be part of the Health System Renewal Advisory Committee, a committee focused on advising the LHIN on the implementation of Patients First, and our strategic priorities.

The South West LHIN’s Patient and Family Advisory Committee joins LHINs across Ontario who are establishing Patient and Family Advisory Committees as part of the Patients First Act, 2016.
