
Access to Care June 12 Webcast
Access to Care is an approach to care focused on supporting people, specifically seniors and adults with complex needs, to live safely in their homes for as long as possible. Over the last year, there have been three streams of work supporting Access to Care in the South West LHIN: Home First, Complex Continuing Care and Rehabilitation (CCC/Rehab), and Assisted Living, Supportive Housing, and Adult Day Programs (AL/SH/ADP).
About Access to Care
Access to Care is an Approach to help people move out of acute hospitals and into other care settings as quickly, smoothly and safely as possible. Access to Care empowers patients/clients to participate in their own care and ensures that they get the right care at the right time and place. It calls for collaboration and integration across the health system.
In the South West LHIN, Access to Care involves three main initiatives:

Resources for Health Care Providers:

Home First is a province-wide strategy and a new way of approaching patient/client care. The central idea is that when a person enters the hospital, health professionals, family members and others work together to get him or her home, if at all possible. Research has shown that this is the best way to manage transitions from hospital.
Benefits of Home First:
For patients and clients, Home First:
- Empowers you to participate in your care.
- Gets you home sooner, minimizing your risk of hospital-acquired infection and functional decline.
- Enables you to live safely and comfortable at home for as long as possible.
- Enables you to make life-changing decisions, such as a move to long-term care, from your own home.
For hospitals, the CCAC and the local health system, Home First:
- Improves the effectiveness of discharge planning.
- Reduces the number of Alternate Level of Care patients.
- Shortens Emergency Department length of stay.
- Provides better access to hospital and long-term care for those who really need it.

Resources for Health Care Providers

As part of the Access To Care Appraoch, Assisted Living, Supportive House and Adult Day Program stakeholders will work together to enhance and realign services to best meet the needs of high-risk clients in our communities.
Residents in the South West will benefit from work done as part of this initiative because available resources will be better provided where clients need them. Instead of having to research available services and compare what can be provided, a CCAC caseworker will act as the client’s personal navigator into the health care system.
The expanded CCAC role will include admissions for these programs so that patients will have a single point of access to local services. This change supports some the major goals of the government’s Excellent Care For All strategy – to reduce unnecessary hospital visits and increase the quality of each patient’s care experience.

Resources for Health Care Providers:

As part of the Access To Care Approach, the Complex Continuing Care (CCC) and Rehabilitation initiative will help ensure these services for frail seniors and adults with special needs are provided consistently and can be easily accessed across the LHIN. After careful study, the existing services will be re-aligned to better meet client needs and provided through a single point of access, the South West Community Care Access Centre (CCAC). Ultimately the work of this initiative will reduce wait times and improve utilization for CCC and Rehab beds, and reduce the number of patients designated as ALC.
