Living Healthy, Independently and Safely at Home

The South West LHIN is Committed to Fulfilling our Vision of an Integrated Health System by 2022

Our third Integrated Health Service Plan (IHSP) helps advance Ontario’s Action Plan for Health Care, works towards fulfilling our Health System Design Blueprint: Vision 2022 (Blueprint) and builds on the accomplishments of the previous two Integrated Health Service Plans. We will achieve our vision through careful planning, resource allocation and collaborative initiatives designed to make strides in transforming our health system.

The plan outlines strategic directions and objectives for 2013-2016, provides overviews of Ontario’s Action Plan for Health Care and our Blueprint and a snapshot of our current health system.

 The IHSP 2013-2016 is our three-year strategic plan developed after a thorough analysis of our health system and identification of gaps and improvement opportunities. It is focused on making a difference in key areas to support people to live healthy, independently and safely at home.

Implementation of our strategic plan relies on collaboration with our health service providers and other partners. The IHSP 2013-2016 outlines numerous collaborative initiatives among hospital, primary care, community-based care and long-term care homes to provide solutions and quality health care.

The Health System Blueprint – Vision 2022

The Blueprint was initiated by the South West LHIN Board of Directors to develop a framework for how the system should be structured across programs and geography, based on a detailed understanding of current services.  It contains a vision of an Integrated Health System of Care to support health care providers in planning for change and finding new ways to work together to meet the evolving needs of all communities in the South West LHIN.
