Our Performance Improvement Approach
The LHIN works with its provider partners to achieve a high-performing health system. But what does a high-performing health system really mean and look like? Improving people’s health care experiences, ensuring people get timely, quality care, and increasing the value received for the money spent on care are markers of knowing if we’ve been successful. Achieving these markers will require leadership, accountability and a culture of performance improvement through a quality lens.
Moving from Strategy (our Integrated Health Service Plan (IHSP)) to Execution (the coordinated set of initiatives and actions taken to implement the plan) is essential and requires a robust performance management approach. Performance measurement, reporting, and quality improvement actions are fundamental elements in the journey to a high-performing health system.
Strategic and operational management tools, such as a scorecard and dashboard, will bring the information together to help inform the discussions on whether the changes being made are making a difference in people’s experiences, quality of care and value for money. Measurement helps determine whether the changes we are making are leading to improvement. It can be as simple as plotting data points over time and looking at the results.
Achieving a high-performing health system will require the attention, focus and dedication of leaders involved in championing and implementing tests of change. The successful change ideas need to be sustained and spread in order to achieve system impact. The Report on Performance is meant to place data and information into the hands of those leaders who can create a culture of continuously pursuing improved quality and efficiency in providing ‘the right care at the right time and in the right place.' The pan-LHIN Health System Performance Indicator Framework is being used to guide our efforts to achieve a balanced set of process and outcome measures. The Framework helps focus on how people move through the health system. The chosen indicators are helping us understand progress, improvement and areas of opportunity. |

The LHIN Report on Performance Scorecard provides a visual and quantitative snapshot of important measures being used to look at the journey to achieve our performance objectives. The scorecard is updated quarterly (posted August, November, February, May). |

The LHIN Report on Performance Tool (May 2013) brings together 60 indicators in one place for the purpose of review, analysis and use by anyone involved in performance improvement. The tool is updated monthly (posted the second Monday of each month).

The LHIN Quarterly Stocktake Report (February 2012) profiles provincial strategies, associated measures and actions that are reported to the public and outlined in agreements between each LHIN and the Ministry of Health and Long-Term Care. The report is updated quarterly (posted February, May, August, November).
