Welcome to the South West LHIN

The South West LHIN is leading the way in transforming the health system to better meet the needs of our communities and to ensure a healthier tomorrow. Transformation means collaborating with health service providers – long-term care homes, community support services and hospitals – in redesigning effective approaches in the delivery of health services and to integrate services for more seamless care. Transformation also means setting performance goals and ensuring accountability in meeting these goals.

To guide this transformation we have a three-year strategic plan, the Integrated Health Service Plan (IHSP) 2013-2016. The IHSP outlines numerous programs and initiatives to help people live healthy, independently and safely at home.  Click here for more information on the IHSP.  On our website you will find information on the progress being made and plans underway to improve the health system for our communities.

Minister's Medal Honouring Excellence in Health Quality and Safety

The Ministry of Health and Long-Term Care has announced the launch of the second annual Minister’s Medal Honouring Excellence in Health Quality and Safety. This annual recognition program aims to honour exceptional work across the health system and showcase successes in cross-sectoral collaboration of health system partners to deliver high-quality, patient-centred care across the continuum.  The award will be presented to one team-based initiative/program and one individual champion at Health Quality Transformation 2014 on November 20. 

Application templates are now available for download from the Minister’s Medal webpage.

Henley Place Opening

The newest long-term care home in the South West LHIN oficially opened on July 9, 2014 in north east London.  The South West LHIN was proud to be a part of the event.
